We use and disclose protected health information (PHI) to treat you, to obtain payment, and to run the practice. We may also disclose PHI when required by law, for public health, or with your written authorization.
You have the right to inspect and request copies of your records, request amendments, request restrictions, request confidential communications, and receive an accounting of certain disclosures. To request records, contact the office where you were seen. We may charge a reasonable cost-based fee as allowed by law.
The public booking form on this website is not a medical record. Do not submit detailed PHI there. Complaints may be filed with our privacy official or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
