Fenix Lumi Optometry (“we,” “us”) operates this website for two California offices. The practice name on this site is Fenix Lumi Optometry. The legal entity may appear as Fenix Lumi, Corp on insurance and contracts. We are an independent doctor of optometry — not Sam’s Club and not LensCrafters.

Information you send us

The public callback form collects only first name, last name, phone, email, which office to call, and optional preferred date and time. That is contact information so we can reach you. It is not tied to a diagnosis, treatment, or workup, and it is not a medical record. Do not include symptoms, medical history, Social Security numbers, or insurance ID numbers. This site is not for emergencies.

Necessary. These keep the site working. They are not used for ads. You cannot turn them off and still have the office memory and display settings persist.

How we use information

We use form details to reach you about an appointment at the office you selected. We use necessary cookies so the site can show the correct NAP, hours, and booking path. We use optional analytics only with consent, to understand which pages and calls people use.

We do not sell personal information. We do not share personal information for cross-context behavioral advertising. Retail partners (LensCrafters, Sam’s Club) are not the practice and do not receive your website form through this site.

California privacy rights

California residents may request to know, delete, or correct personal information, and to opt out of sale or sharing. Because we do not sell or share personal information as those terms are used in CPRA, the cookie banner is an extra control for optional analytics and maps — not a “sale” toggle.

Health information in the clinic

Health information collected in the exam room is governed by our HIPAA Notice of Privacy Practices, not this marketing-site policy. Do not send detailed PHI through this website.

Children

This website is for adults booking care, including pediatric exams typically from about age 5. We do not knowingly collect personal information from children through this marketing site.
