Do I need a Sam’s Club membership to see the Chino doctor?

You may need a Sam’s Club membership or day pass to enter the club. Fenix Lumi is an independent optometry office inside Sam’s Club Optical — we do not sell memberships. Call us if you need visit instructions; some patients are able to tell the door they are here for the eye doctor.

Are you Sam’s Club or LensCrafters?

No. Fenix Lumi is an independent doctor of optometry. LensCrafters (Moreno Valley) and Sam’s Club Optical (Chino) provide space and frames. We provide the exam and medical eye care.

Can I start in Chino and finish specialty care in Moreno Valley?

Yes. Comprehensive exams, contacts, and medical visits can begin in Chino. Advanced dry eye (OptiLIGHT IPL, OptiPLUS RF) and specialty / scleral lenses are at Moreno Valley — we can schedule you there.

How do I change locations on the site?

Use the location pill in the header (“Change location”). Your phone number, hours, map, and booking will follow the office you pick. We save the choice in a necessary cookie named fenix-lumi-location.

How do cookies work on this site?

A banner lets you accept cookies or decline optional cookies. Necessary cookies remember your office and light/dark display. Optional cookies are only for anonymous usage events and the interactive Google Map. Change your choice anytime under Cookie settings in the footer, or read the Privacy Policy.

What are your Moreno Valley hours?

Monday closed; Tuesday–Friday 10:00 AM–6:30 PM; Saturday 10:00 AM–6:00 PM; Sunday 11:00 AM–5:00 PM. Published sources vary slightly — please confirm before your visit.

What is the Chino phone number?

What are the Chino doctor hours?

Doctor hours are not the same as Sam’s Club Optical retail hours. Optical is often Mon–Sat about 10am–7pm with a shorter Sunday; Fenix Lumi exam hours must be confirmed with the office. Use the online scheduler or the callback form and we will reach you.

How much is an exam without insurance?

At Moreno Valley, cash starting fees (insurance may apply): exam about $84.95; contact lens exam/fitting about $139.95; retinal imaging about $39; dilation about $15. Chino fees: confirm with the office.

What is the minimum age for children?

Pediatric exams are typically offered from about age 5. Call if your child is younger so we can advise.

Do you perform LASIK, cataract surgery, or Light Adjustable Lens procedures?

No. Fenix Lumi is an independent optometry office. We examine, discuss candidacy, and co-manage with a surgeon. Laser vision correction, cataract surgery, YAG, and Light Adjustable Lens implantation happen at a surgical center — not inside LensCrafters or Sam’s Club. Do you prescribe presbyopia (near vision) eye drops?

Some adults are candidates for prescription near-vision drops such as pilocarpine (Vuity-type treatments). They are not for everyone and can affect night vision. We decide after an exam. Read the presbyopia eye drops page in Eye Health, then book.

How do I schedule online?

Online scheduling is for the Chino office only (patient portal). Scan the Chino QR or use Book Chino. Moreno Valley does not use that portal yet — use Book Moreno Valley or the callback form on /book. The portal is not for emergencies and not a medical record.

Is the booking form for medical records or emergencies?

No. The public form collects first name, last name, phone, and email so we can call you back. It is not a medical intake and does not collect health information. This site is not for emergencies.
